Iontophoresis Literature Review

 

Iontophoresis is an electrical stimulation that is a therapeutic modality often used in physical therapy. While different medications are used in iontophoresis, depending on the condition being treated, dexamethasone is commonly used.(1)


A study that included 82 patients with epicondylitis (tennis elbow) compared the injection of dexamethasone with the delivery of dexamethasone via iontophoresis on patient outcomes. Outcome criteria included pain, grip strength, function, and return-to-work status. The results stated: “The iontophoresis patients had statistically significant improvement in grip strength at the conclusion of hand therapy compared with baseline. They were also more likely to get back to work without restriction. This study suggests that this iontophoresis technique for delivery of corticosteroid may be considered a treatment option for patients with lateral epicondylitis.”(2)


A study conducted at a pediatric rheumatology center assessed the safety and efficacy of dexamethasone iontophoresis for the treatment of temporomandibular joint (TMJ) involvement in children with juvenile idiopathic arthritis. Children and adolescents with juvenile idiopathic arthritis are prone to TMJ arthritis. “Besides pain, swelling, and limitation in the range of motion, TMJ arthritis can present with headaches, neck pain, and pain with mastication. If untreated and persistent, TMJ arthritis can result in micro-retrognathia, facial asymmetry, and therefore, decreased quality of life.”(3)


The study results showed that 69 percent of patients who received dexamethasone iontophoresis experienced statistically significant improvement. Resolution of pain occurred in 73 percent of the patients. The study concluded, “In this pilot study, DIP (dexamethasone iontophoresis) appeared to be an effective and safe initial treatment of TMJ involvement in juvenile idiopathic arthritis, especially among patients with decreased TMJ measurements.”(3)
